Criteria: Individuals or organizations that have demonstrated exceptional ability to make change happen in healthcare facility design.

Who is Eligible: Nominees can be, but are not limited to, design professionals, design firms, product manufacturers, healthcare professionals (including practitioners), healthcare institutions, healthcare facility owners, researchers, educators, authors, consultants, associations, association leaders, etc.

What to Send:

• The individual or organization's full name, title, address, telephone, e-mail, website.
• A description (200 words or less) of why this individual or organization is a changemaker.
• Resume or list of accomplishments, if available.

How to Send: E-mail as an attachment or as text in the body of an e-mail message to Angela Skaggs, Project Manager, The Center for Health Design.
