Changemaker Award

NOMINATE A CANDIDATE TODAY FOR THE 2012 CHANGEMAKER AWARD


Do you know an individual or organization that has made a difference in the world of healthcare facility design? Nominate them for The Center for Health Design’s 2012 Changemaker Award! The recipient of this award will be announced at the 2012 HEALTHCARE DESIGN conference, November 3 - 6, 2012, in Phoenix, Arizona.


AWARD SUBMISSION INFORMATION

Criteria: Individuals or organizations that have demonstrated exceptional ability to make change happen in how healthcare facilities are designed and built, and whose work has had broad impact.


Eligible Nominees: Nominees can be, but are not limited to, design professionals, design firms, product manufacturers, healthcare professionals (including practitioners), healthcare institutions, healthcare facility owners, researchers, educators, authors, consultants, associations, association leaders, etc.


Nomination Deadline: March 9, 2012

What to Send:


• The individual or organization's full name, title, address, telephone, e-mail, and website address.
• A description (200 words or less) of why this individual or organization is a changemaker.
• Resume or list of accomplishments, if available.
