Thank you for your interest in becoming a member of The Center for Health Design's Community of Affiliates. Your support helps us continue our efforts and activities to improve the quality of healthcare through building design.

To apply for a one-year membership, please select your affiliate level below and click the Submit button to proceed to payment processing. (Note: Before you can submit and pay for your affiliate membership, you must log in; if you do not already have an account, you can create one here.)

If you wish to donate another amount within the suggested range, or to pay for your membership by mail (check or credit card), download our application in PDF format and fax or mail it to us using the information found at the bottom of the document.

Once we receive and process your application and payment, you will receive a confirmation letter, affiliate pin and any other benefits based on your membership level. Your membership will be up for renewal one year from when we process your application and payment. If you have any questions, please contact us.
